COVID-19 Care Plan for Patients
You can use the COVID-19 care plan for staff to track their home self-testing and to provide statistics for NHS England. Ask your Success Team Member for help in setting this up in your hospital.
What’s contained in a care plan?
PKB will always show any diagnoses, allergies and medications that have been entered in the patient record. The main body of the plan includes an action plan. The action plan can contain information on the patients care, health goals and advice on monitoring their conditions. (Including video or web links). Key Blood Tests, Symptoms or Measurements can also be selected too tracked and displayed for the patient.
How can I provide the care plan to my patients?
In a public health crisis PKB can work with your organisation to act at speed and scale to mass register patients and provide them with key tools for remote care. Care plans can be uploaded once by your Team Co-ordinator to be instantly available for clinical staff to add to a patients record. Patients can also add their own care plan templates to their record.
COVID-19 Information and Advice Care Plan
This care plan provides advice on how to reduce the chances of catching Covid, get a test, and self-isolate. This template was created from the UK government advice from the 5 November 2020. The screenshots show the care plan in mobile and desktop versions. Below is the source code for the PKB template to use with your patients.

<p>On the 5th November, the UK Government introduced new restrictions regarding COVID-19. <p>There are three simple actions we all should take to continue protecting ourselves and each other from COVID-19:</p>

<li style="list-style:inherit; margin-left: 15px;"><b>Cover face</b> - wear a face covering in enclosed spaces</li>
<li style="list-style:inherit; margin-left: 15px;"><b>Make space</b> - stay at least 2 metres apart - or 1 metre with a face covering or other precautions.</li>

<h2>The main symptoms of coronavirus (COVID-19) are:</h2>
<ul>
<li style="list-style:inherit; margin-left: 15px;"><b>A high temperature</b> - this means you feel hot to touch on your chest or back (you do not need to measure your temperature)</li>
<li style="list-style:inherit; margin-left: 15px;"><b>A new, continuous cough</b> - this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than usual)</li>
<li style="list-style:inherit; margin-left: 15px;"><b>A loss or change to your sense of smell or taste</b> - this means you've noticed you cannot smell or taste anything, or things smell or taste different to normal.</li>
</ul>
<p>Most people with coronavirus have at least 1 of these symptoms. If you display one or more of these symptoms you should <a href="https://www.gov.uk/get-coronavirus-test" target="_blank">apply for a COVID-19 test</a>.</p>

<h2>Who can get a free test:</h2>
<p>You can only get a free NHS test if at least one of the following applies:</p>
<ul>
<li style="list-style:inherit; margin-left: 15px;">you have a high temperature</li>
<li style="list-style:inherit; margin-left: 15px;">you have a new, continuous cough</li>
<li style="list-style:inherit; margin-left: 15px;">you've lost your sense of smell or taste or it's changed</li>
<li style="list-style:inherit; margin-left: 15px;">you've been asked to by a local council</li>
<li style="list-style:inherit; margin-left: 15px;">you're taking part in a government pilot project.</li>
</ul>
<p>You can also get a test for someone you live with if they have symptoms.</p>

<h2>How to self-isolate if you're asked to:</h2>
<p>If there is a chance you could have coronavirus, you may be asked to stay away from people (self-isolate). You may also be asked to self-isolate if someone in your household or bubble has symptoms of coronavirus.</p>
<p>This means you should:</p>
<ul>
<li style="list-style:inherit; margin-left: 15px;">stay at home</li>
<li style="list-style:inherit; margin-left: 15px;">not go to work, school or public places</li>
<li style="list-style:inherit; margin-left: 15px;">not use public transport or taxis</li>
<li style="list-style:inherit; margin-left: 15px;">ask friends, family members or delivery services to do errands for you</li>
<li style="list-style:inherit; margin-left: 15px;">try to avoid visitors to your home - it's ok for friends, family or delivery drivers to drop off food.</li>
</ul>
<p>You may need to do this for up to 14 days to help reduce the possible spread of infection.</p>

<h2>Stay safe - NHS COVID-19 app</h2>
<p>The free NHS COVID-19 app is a vital part of the NHS Test and Trace service in England, and the NHS Wales Test, Trace, Protect service.</p>

<p>The new NHS COVID-19 app, now available to download for free in England and Wales, is the fastest way to see if you're at risk from coronavirus. The faster you know, the quicker you can alert and protect your loved ones and community.</p>
<p>The app has a number of tools to protect you, including contact tracing, local area alerts and venue check-in. It uses proven technology from Apple and Google, designed to protect every user's privacy. </p>

COVID-19 Symptom Tracking Diary Care Plan
This care plan allow a patient to track the symptoms of Covid-19. The instructions advice the patient what to do if their symptoms suggest they might have Covid-19. The screenshot shows the care plan in desktop versions. Below is the source code for the PKB template to use with your patients.

<p>This plan is designed to help you record and monitor your health and track any symptoms you may be experiencing. It will also provide information on what to do if you are unwell. </p>
<p>Please record your symptoms daily using the 'Edit plan' button in the top right hand corner of this page. Tick the symptoms that apply to you on that day and click the 'Save' button at the bottom of the page. Each time you update the plan by adding your symptoms, you are building a picture of how you are and whether you are improving or getting worse.</p>

<h2>What to do if you are unwell:</h2>

<p>Stay at home and <a href="https://www.nhs.uk/conditions/coronavirus-covid-19/self-isolation-advice/" target="_blank" title="NHS advice on self isolating">self-isolate</a> if you begin to feel unwell, even with mild potential COVID-19 symptoms (such as a headache and slight runny nose) for 7 days. <b style="color:red;">Do not go to a GP, pharmacy or hospital.</b></p>
<p>Why? Avoiding contact with others and visits to medical facilities will help protect you and others from COVID-19 and other viruses and also allows these facilities to operate more effectively.</p>
<h2>What to do if you need medical help:</h2>
<p>If you have symptoms of COVID-19 (a high temperature or new continuous cough), use the <b>online</b> <a href="https://111.nhs.uk/covid-19/" target="_blank" title="See the NHS 111 Coronavirus information">NHS 111 Coronavirus Service</a>.</p>
