PREOM Consultation
Key objectives
To use PKB to facilitate remote communication and monitoring with patients. By providing patients with a digital Patient Reported Experience and Outcome Measure questionnaire that they could do at home before appointments.
Outcome measures
To equip patients with a tool to support self-management of their health and wellbeing, as well as providing a facility to interact digitally rather than relying on a face to face consultation.
Current Baselines
All Patient Reported Experience and Outcome Measure are completed face to face on paper and patients do not get copy to take home.
Workflow
Team: send patient consultation
PKB: notifies the patient of the consultation
Patient: completes consultation and submits to the team for review
PKB: notifies the team, the consultation has been completed
Team: review in appointments with the patient
